Crisis Peer Support

Workload Management: ·       Assists the Crisis Clinician with the crisis evaluation process, including determining the capacity of the client's natural supports to assist the client or identifying subtle risk factors. ·       Schedules and conducts timely follow-up interventions, as clinically indicated, to maintain client stabilization and continue to follow-up until the client is either stable, seen within community behavioral health services or 30 days has elapsed since the first intervention.  ·       As appropriate, provides own life experience to engage individuals in crisis, assist the individual and/or family with psychosocial supports, and development of an individualized recovery plan. ·       Promotes client self-advocacy, self-care, and illness management, wellness and recovery. ·       Completes documentation and participates in team meetings. ·       Submits weekly timesheet reflecting hours worked and time off accurately and within stated deadline. Quality of Work: ·       Provides crisis interventions (initial and follow-up) with the overarching goal to maximize the client’s potential to remain within the community through the use of natural supports and other mobilized supports. ·       Provides follow-up crisis interventions with the overarching goal to maximize the client’s potential to remain within the community through the use of natural supports and other mobilized supports. ·       Provides follow-up interventions in an environment/location that is preferred by the client and/or his/her family.  ·       Assists Crisis Clinicians with developing a positive Safety Plan when individual are to remain in the community as their disposition. ·       Provides empirically supported psychoeducation and equivalent support to client and collaterals. ·       Complies with agency health and safety rules that may include attendance in safety training(s), participate in drills, familiarity with workplace practices, and maintain a safe work area. Training Outcomes & Development: ·       Maintains current knowledge of community resources as demonstrated by timely and effective referrals to address post-crisis client needs. ·       Participates in all state mandated training and re-certification, based on prevailing state requirements. ·       Participates in all training as represented in the employee's most recent training/development plan. ·       Accesses and utilizes agency resources on the Intranet to include electronic health record, payroll, timekeeping, benefits, policies, etc. Communications & Customer Service: ·       Develops and maintains collaborative working relationships with hospital-based staff and other community providers in order to effectively serve all clients in crisis. ·       Completes comprehensive, same-day clinical documentation of all assessments and crisis interventions. ·       Represents the agency effectively and conducts themselves professionally within meetings with internal and external participants. ·       Provides courteous, respectful and prompt service to both internal and external customers at a quality level that meets or exceeds the standards set forth by the Policy #214 Customer Service Policy. ·       Regularly attends all required meetings and participates in a manner that is value-added to the specific meeting objectives. Initiative and Engagement: ·       Engages clients in the physical environment of the client and/or family’s preference that supports successful crisis interventions and, as necessary, offers alternative options to the client and/or family. ·       Informs supervisor of questions and concerns and communicates possible recommendations and solutions. ·       Follows agency policies and procedures and best-practice standards. Professionalism/Leadership: ·       Demonstrates through example recovery-oriented, problem-solving strategies. ·       Promotes positive relationships and work environment through courteous, clear and effective communication. ·       Provides courteous, helpful, timely, and respectful service to those they serve, both within West Central and to clients and members of the community at large. ·       Maintains client, employee and agency confidentiality standards as outlined in Policy #208 Confidential Information Policy. Essential Job Requirements: Education: High School Diploma or GED equivalent, OR lived experience Experience: 1-2 years of relevant experience, either personally or professionally Required Skills: Excellent organizational and communication skills Excellent customer service skills Excellent computer skills and knowledge of Microsoft Office Ability to learn necessary computer and technical skills essential to the position and agency Preferred Skills: Knowledge of and experience with electronic health record Prior experience in a community mental health environment Physical Requirements: Ability to access all sites Ability to life, push and/or pull 10 lbs. Other: Ability to obtain and maintain agency and/or State of New Hampshire required background check(s) High School diploma or GED equivalent OR Lived experience Excellent customer service skills Excellent organizational and communication skills Be a part of our team!
