Are you passionate about helping others build independence and well-being? Averte is seeking a dedicated Case Manager to guide residents through their recovery journey by coordinating care, building therapeutic relationships, and supporting life skill development.
?? What You’ll Do:
• Develop and monitor individualized treatment plans
• Facilitate resident onboarding and discharge planning
• Coordinate medical, dental, and vision appointments
• Track progress and maintain accurate documentation
• Collaborate with a supportive interdisciplinary team
? What You’ll Bring:
• High school diploma or equivalent (Associate’s degree preferred)
• 2+ years’ experience in human services or mental health
• Strong communication, organization, and computer skills
• Valid chauffeur’s license and insurable driving record
